Need a copy of your child's immunization record? Click Here. Must have name, date of birth, and a Social Security or Medicaid number.

There is a $15.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics.  We accept cash, check, credit or debit cards for payment at this time. In addition, we are unable to accept children with Hawk-I coverage.  These children should be taken to their physician for immunizations.

I-Smile™ Oral Health Program PDF Print E-mail



The I-Smile™ program’s mission is to increase the number of children and pregnant women in Iowa with a dental home.

Children ages 0-21 and pregnant women qualify for I-Smile™ services.


I-Smile™ Services Provided

  • All services provided are free of charge
  • Dental screenings and fluoride varnish applications
  • Oral hygiene and dental nutritional counseling
  • Completing school dental screening certificates
  • Care coordination for families such as:
    • Finding a dentist
    • Understanding or acquiring dental insurance
    • Identifying other payment options
    • Scheduling dental appointments
    • Answering general oral health questions
