Volunteer for the MRC

SDHD is currently looking for both medical and non-medical individuals who are interested in volunteering for the Medical Reserve Corps (MRC). Individuals will help our community prepare for, respond to, and recover from a variety of public health emergencies and other community events. IMMUNIZATIONS

Need a copy of your child's immunization record? Click Here. Must have name, date of birth, and a Social Security or Medicaid number.

To better serve our clients, effective April 1, 2014 all immunizations for infants, children and adults will be by appointment only. SDHD will no longer offer walk-in children's immunization clinics. This transition will reduce wait
times for individuals and families and allow for more personal services. To schedule an appointment, call 712-279-6119 and ask for the clinic.

All clinics held at SDHD, 1014 Nebraska St.

There is a $10.00 administration fee per child for all non-Iowa Medicaid children receiving vaccinations through our immunization clinics.  We accept cash, check, credit or debit cards for payment at this time. In addition, we are unable to accept children with Hawk-I coverage.  These children should be taken to their physician for immunizations.

Testing for drugs of abuse is done via urine samples.

LOCATION:  Siouxland District Health Department laboratory

HOURS:  8:00 AM - 4:30 PM

APPOINTMENT:  Testing is done on a walk-in basis, no appointment is necessary.

COSTS:  $15 for directly observed sample collection and $10 per drug tested.  Discounts for multiple drug panels may be available.  Cash, debit cards, or money orders will be the only methods of payment accepted.  No checks will be accepted.

AVAILABLE DRUG TO BE TESTED: Marijuana, amphetamines, methamphetamine, cocaine, morphine*, benzodiazapines**, and K-2***.

Test results can often be obtained the same day as collection.

* - Morphine testing may detect substances such as codeine, hydrocodone, oxycodone, percodan, and demerol.

**- Benzodiazapines testing can detect substances such as valium, librium, xanax, versed, halcion, and restoril.
