ESSENTIAL FUNCTIONS:

Responsible and accountable to provide a broad range of services to clients in the home setting according to policies and procedures of Siouxland District Health Department, with the goal of achieving and/or maintaining client independence and to reduce, prevent or delay inappropriate institutionalization of clients.  Direct Care Workers are trained and supervised individuals who provide services, care and emotional support to clients.  They work in the Personal Care Only/Homemaker Program (PCO/HM) serving Woodbury County clients in their homes Monday through Friday 8:00-4:30pm. Reports directly to and responsible to the Personal Care Only/Homemaker Program (PCO/HM) Program Manager, and the Nursing Director.

APPLICANT MUST MEET THE FOLLOWING REQUIREMENTS:

Must possess H.S.Diploma or GED.  75-Hour Certified Nurse Aide course with Course Transcript that shows successful completion of 75-hour Certified Nurse Aide Course.  Must be able to communicate and work well with diverse population groups.  Knowledge and ability to provide needed in-home care to clients, documenting and reporting in accordance with the regulations and requirements of responsibilities.  Physical ability to lift/transport up to 50 pounds

Candidates for hire must successfully pass a background check, a physical exam and drug screening test prior to employment. Must have valid Driver License, ability to obtain car insurance at agency-specified liability level and reliable transportation.  CPR Certification required or willing to attend training.
