Siouxland District Health Department's Health Maintenance Program provides services to individuals with a stable chronic condition in their home. Individuals must be Woodbury County residents, meet financial criteria and follow a sliding fee scale.  

Nursing Program

• Medication management to include instruction and med set up as needed
• Administration of injections and blood draws
• Teaching and nursing interventions
• Maintain stability of chronic conditions
• Coordinate with physicians, pharmacies, caregivers, and other community agencies
• Physical, emotional, and home safety assessments
