Siouxland District Health Department (SDHD) is committed to providing equitable access to all programs and services. Individuals who would like to request a reasonable accommodation or information in alternate formats are encouraged to do so when making an appointment. SDHD will make every attempt to fulfill the request if provided at least two working days' notice, but providing at least a two-week notice will allow us to provide seamless access.
