Homemaker/Home Care Aide Program Manager

ESSENTIAL FUNCTIONS:

Responsible for care coordination and service management for the Home Care Aide and Homemaker Programs. In accordance with state regulations supervises all direct care workers, assesses client’s needs for service, client financial assessments, develops plan of care, written assignment of duties including frequency of task performance and length of visits, assignment of worker to case, supervision of the home care aide’s performance, review of home care aide’s progress notes, appropriate referrals, appropriate reassessment activities, and other duties as assigned.

APPLICANTS MUST MEET THE FOLLOWING REQUIREMENTS:

Licensed Practical Nurse (LPN) licensed to practice in the state of Iowa; OR Bachelor’s degree in Social Work, Sociology, Family and Consumer Science, Education or other related human services field; OR Registered Nurse (RN) licensed to practice in the state of Iowa OR two years of related public health experience. Supervisory and program management experience preferred. CPR Certification and Mandatory Child & Adult Abuse Reporter Certification required or willing to obtain; computer skills needed; ability to work with diverse populations and to work independently in an unstructured setting with minimal supervision; physical ability to transport essential supplies and to lift up to 50 pounds.
